MEMORANDUM FOR RECORD
SUBJECT: COVID-19 Prevention and Response Protocol
The purpose of this memo is to outline the procedures for preventing COVID-19 and responding to positive COVID-19 cases to ensure the safety of Point Avenue (hereby referred to as “the Company”), our Centers, and all team members. Article 1: Response Team
The PAT COVID Response Team will be responsible for managing COVID-19 related information, communicating protocols and processes, and managing COVID cases that rise within the center.

Article 2: Definition
– Suspected F0: person experiences any of the following symptoms:

Fever > 38C (100.4 F) –
Cough –
Tiredness –
Loss of taste or smell –
Nasal congestion
– F0: confirmed positive result on the ATK
– F1: Person who is suspected of being infected, due to having direct contact with an F0/ or other F1 – PA: Employer – TM: Team member – ST: Students – WFH: Work from home

PROCEDURES
1.
Stage 1 (APPLIES TO ALL): Suspected F0 in the Company
When team members have symptoms such as severe cough, fever, etc. or been at risk of Covid exposure, they must immediately follow the outlined steps:

Step 1: Conduct an ATK Test. –
If the RT result is positive, please report to your GM/HR/Direct Supervisor and proceed to Step 2 –
If the RT result is negative, take a second test and continue to monitor symptoms. –
Inform your direct supervisor, HR, and the GM concerning your symptoms.

Step 2: When you’ve tested positive, HR/GM will share your situation with everyone in the team. You’re obliged to report who were the TMs you had been in contact with so they are aware and test themselves. –
If you have mild to no symptoms, you are required to stay at work with DMHT defensive measure; –
Distancing : Keep social distance for other TMs for at least 1 metre. Have your meal separate from others. –
Mask wearing : Wear the mask at all times except when eating or drinking. –
Hand washing : Frequently wash your hands with hand sanitizer or soap. –
Testing : Regularly monitor your symptoms (check temperatures, coughing etc.) –
If you feel unwell, please inform your direct supervisor, HR, and the GM and take days off until you feel better. –
Quarantine is not required. After the symptoms are gone, you will be back at work with DMHT defensive measure.

Stage 2 (APPLIES TO MANAGEMENT): Confirmed F0 in the Company
Upon confirming that a team member is an F0, Company and Center-level leadership must implement the following emergency response plans.

Notify all TMs and STs who were in close contact with F0 without a mask for more than 5 minutes within the range less than 2 metres. –
Provide the ATK tests for them. –
F1 are required to wear a mask on and keep social distance for at least 1 metre to other TMs.

Conduct cleaning and disinfection of the Center and other working areas with reported COVID-19 case(s).
